French decree no. 2007-435 of 25 March 2007 sets a precise framework. Manipulation of the cervical spine, and manipulation of the skull, face and spine in infants under six months, may only be carried out after a medical diagnosis confirming the absence of contraindication.
In practice: if your complaint calls for cervical manipulation and you have no prior medical opinion, I work with alternative techniques: they exist and they are effective. I do not cut corners with this framework.
How many sessions to expect
For recent mechanical pain with no particular history, one or two sessions are usually enough. Pain that has been present for several months, or that returns cyclically, generally calls for two to four sessions spaced two to three weeks apart, so the adaptations have time to settle.
If nothing has shifted after two sessions, booking a third on principle is not the answer: it is a sign the diagnosis needs revisiting. I will tell you so.

Does structural osteopathy hurt?
No. Structural techniques are quick and low-amplitude; they should not be painful. A stiffness similar to post-exercise soreness may appear in the 24 to 48 hours after a session and resolves on its own.
Does it have to crack to work?
No. The joint sound is a gas phenomenon with no therapeutic value in itself. A gentle mobilisation can restore the same mobility. If you would rather avoid thrust techniques, say so and I will adapt.
Can I book without a referral for back pain?
Yes, osteopaths are directly accessible in France and no prescription is needed. However, pain accompanied by fever, loss of strength, urinary problems or unexplained weight loss requires a medical opinion before any session.
